Unusual post dosing reactions with Methotrexate

A few patients have a wide array of symptoms within 24 hours of taking Methotrexate. These include—
1) Fatigue, generalized weakness

2) Increase in the joint pain

3) Generalized muscle ache

4) Lightheadedness, brain fog (feeling strage, fuzzy, cobwebs)

 
These symptoms are uncommon but very much real & related to Methotrexate.

How can these symptoms be taken care of?
1) Change from subcutaneous/ intramuscular injection to oral tablets.

2) Dividing the dose over 2 days.

3) Reducing the dose.
